Training Client Inquiry Questionnaire

For every training client or fitness/coaching inquiry, it is essential that I gather as much information on you as I possibly can. That way, I can better understand all the variables that could affect your fitness. After I review the written responses, we will do a phone call. Before our phone call, please have the following questions answered so that I may be able to give you the most accurate information possible. I will respond in writing before the phone call if I need additional information. Please feel free to make a list of any questions you may also have for me and add them to your response here ideally or if not, you can bring them up during the phone call.
